Bring better notes. Leave with a clearer next step.

A printable conversation sheet for appointments, support visits, or the next call you have been putting off.
A useful appointment note does not need to tell your entire life story. It needs to help you remember what you wanted to discuss and what you agreed to do next. This simple format works on paper or in a private note on your own device.
Before you go: pick three questions
Write the reason for the visit in your own words. Under it, list up to three questions in order of importance. Put the hardest-to-remember question first. Add any information or documents the office specifically asked you to bring, and check the appointment details.
If you want someone to join you, check the office’s visitor or telephone participation arrangements beforehand. Tell that person whether you want them to take notes, help you remember a question, or simply be there.
Your conversation sheet
- Date, place, and person I am meeting
- ________________________________________________
- The main thing I want to discuss
- ________________________________________________
- My three questions
- 1. ______________________________________________
2. ______________________________________________
3. ______________________________________________ - What I heard and want to confirm
- ________________________________________________
- Next step, responsible person, and timing
- ________________________________________________
Before the conversation ends
Read back your understanding in ordinary language: “My next step is this, and I should contact this office if I have a question. Have I understood correctly?” Ask where written instructions or a visit summary will be available. If something is unclear, make that the next question rather than filling the gap yourself.
Keep the note useful afterward
Put follow-up actions on your calendar and store the note with the relevant paperwork. Avoid sending medical details, identification numbers, or benefits documents to a retail contact form. This page does not collect your notes and is an organizational worksheet, not medical or legal guidance.
